Question Narrow shoulders

Hi all
I've been making sleeveless dresses recently, from commercial patterns, but am having loads of problems fitting the tops. I bought the Lutterloh system with a view to using that but, to cut a long story short, I believe it is my narrow shoulders which are causing some of the problems.
My individual shoulder length is only 4.5 inches which is about an inch smaller than the patterns. How do I alter the pattern? Do I just chop the end of the shoulder off and reshape the armhole or do i hae to put a dart or something in? I also have recurring problem with the back neck standing proud of my body. Can anyone help, please?
I`m getting really frustrated now I've tried tissue fitting but can't do it by myself
